A member of the Toorak Basketball Club includes a player, a parent or guardian of a player, a Coach, Team Manager or Volunteer

Reason for completing form

Complaint: A general expression of dissatisfaction with a situation or the behaviour of another


Grievance: A specific and serious feeling of wrongdoing related to a breach of the code of conduct, harassment, discrimination or vilification by individuals


Incident: a distinct action or occurrence that relates to a code of conduct breach, injury, accident, safety or child safety breach.

Please include all relevant information a detailed description of events such as what occurred, dates, times and people involved or anything else you feel is relevant
